Energy transformation is to change energy from one form to another: potential energy to kinetic energy, heat energy to electricity etc.

When water falling from a height drives a turbine, first the potential energy of the water (by being at a height) is converted to kinetic energy which is transferred to the turbine blades and that in turn is used to produce electricity by rotating the generator's rotor in a magnetic field.

When a chemical reaction takes place (like the burning of carbon to produce carbon dioxide), energy may be released. Here too, it is the conversion of a chemical potential energy to a thermal energy. When a rotor of a generator rotates producing electricity, the mechanical energy is converted to electrical energy. When nucler fusion (like what is taking place in Sun) released energy, it is the disappearance of mass which is converted to energy (E=m.c^2). Same case with nuclear fusion too. When a high energy Gamma Ray (which is basically electromagnetic radiation) disappears spontaneously creating a pair of electron and positron, it is the transformation of energy into matter.

Please remember that energy can neither be created nor destroyed. It can only be converted from one form into another. After the phenomenon of pair production and energy production through nuclear fusion and fission were discovered, the law of conservation of mass and law of conservation of energy were integrated into a single law: The law of conservation of mass and energy. Since the universe is apparently a closed system (atleast as per the present day science), the total mass and energy of the universe is constant and only a transformation is taking place. However, scientists are now talking of dark matter and dark energy to explain some phenomena which cannot be explained with the known mass and energy of the universe.
