Im 16, im skinny & kinda unhealthy.
So i was thinking of building my body till healthy enough, and also preparing for national service.

But if i was to work out while im skinny, i'd look like a stick with muscles?

So i was thinking if its ok to gain like 20lbs then workout, then my body would look more proportional. currently im 102lbs at 5'4"

What do you think?

I think you should increase what you eat but not with unhealthy foods.
Eat lots of fruit, vegetables, wholegrain breads, rice, pasta, cereals, raw nuts and seeds, yoghurt, milk, eggs, etc
Also when exercising incorporate weights, cardio, etc to build muscle too.
Dont go overboard on eating fatty foods like doughnuts, pastry, chocolate, ice-cream. you will not benefit from this.

It is better to gain the weight by working out instead of just putting on weight.
