As usual michinoku2001's got my vote.
I recommend especially the Matsushima area.
The scenery is beautifully Japanese.

Also Yamagata is a nice place to visit as well.
Of course winter is beautiful but freezing cold.
Snows like crazy.
I've actually gagged because so many snow flakes would fly into my mouth while I talked and walked at the same time. It's funny but frustrating.

I also enjoyed Ishinomaki.
Great seafood in this area.

As for Hokkaido area, I've only been to Sapporo and Hakodate.
I din't find Sapporo as fun as Hakodate.
Hakodate has some fun things you can do and see.
And again the seafood here is also just phenomenal.
I recommend this place called "Ika Ika tei" if you like Uni and Ikura and crabs. They got these Ikura-don and Uni-don like you wouldn't believe.

Some good places to visit in Tohoku are Hiraizumi-"the Nikko of the North", Matsushima/Sendai, Yamadera(a very cool mountain temple complex) and Aizu-Wakamatsu.
