Are there any rules that prevent me from leaving the company while on medical disability?

You can leave your current job at any time, but once you do, you will no longer be eligible to receive disability benefits.

Also, depending on the specific laws in your area, you probably won't be able to collect unemployment benefits if you voluntarily resign.

You need to check with your state's disability agency for the answer to this one. Presumably you are on medical disability because you cannot perform your current job. If you leave the company, I believe you can collect unemployment, but no longer can collect disability, since technically, the company you are currently with is paying for it. A human resources person would know more.
