For your own protection, you may want to have the document notarized, though this is usually not necessary. Though nobody is allowed to walk through the building I work in without signing an NDA, we have never gone to the extreme of requiring notarization ourselves.

This is what makes it what it is. I use non-disclosures on a daily basis in my business and we never use the same one. This gives us the chance to add or not include anything we want.
