I want to fill out a application for disability from the social security administration.

Don't believe one can apply onLine, but if the applicant takes plenty of govt. identification to their local Social Security office, the workers there will know exactly what to do for you. It usually begins with PROVING that you are disabled, and usually through govt. referred doctors for examination who complete the forms the SS office sends with you to give to the doctors, then the doctors are to mail the forms in themselves. ALWAYS READ and FOLLOW DIRECTIONS AND KEEP COPIES OF EVERY SIONGLE PIECE OF PAPER. Most applicants are denied on the first application. Do not give up if you are sincerely disabled should you be denied. If you receive a denial, immediately go BACK to the SS office and REAPPLY, which is usually not denied. However, if denied over and over, simply continue to REAPPLY. They eventually accept most applicants who are persistant. Be certain to take a birth certificate, driver's license, SS card, and several forms of mail at your mailing address. Be patient and kind, prepared to wait for whatever length of time it takes, and for however many reapplications it takes. Then, if after many applications you are STILL denied, there are attorneys who specialize in taking your case on a contengency, (without pay, because the govt. actually pays them to represent you, oddly enough.)

I'll just bet that if you pick up the phone and call your local social security office they not only can tell you how to do it but will have forms and everything.
