I don't know if you'd consider this a fashion disaster, but having my hair dyed a coppery red when I was 16 was pretty horrid. I looked AWFUL. You'd think the colorist would have talked me out of it!
Other notable fashion disasters: My pink acid-wash jeans and matching jean jacket circa 1987, my giant red Sally Jessy Raphael-esque glasses, and pretty much any other horrific '80s fashion trend suitable for pre-teens at the time.

A hideous perm that seemed to last the duration of the late 80's accompanied by a peach blouson style jacket you rolled the sleeves up on Miami Vice stylee. Truly horrendous. I can't blame my mum, i picked them all by myself. She was the one with the all in one peach jumpsuit, maybe i got off lightly!

White socks. It was acceptable in Kansas when I was in Junior High, but definitely out in Ohio. When we moved, people kept asking me if I was from Parma no matter how often I reminded them I was from Kansas.
I had to watch Big Chuck and Hoolihan one Friday night to realize what they were saying.
Ironically, right before we moved, some kid from the East coast started at my Kansas Jr High. His first day of school, he wore a white shirt and white bell bottoms and had shoulder length hair. The crowd would part as he walked down the hall everyone was so stunned. It didn't dawn on me that I might soon be as much out of place as he was.
Edit: I find Donna T's response very perplexing. Why would you have to get dressed in the dark? Did you break into someone's house and have to grab some clothes from the closet? (It would make a better excuse, "These aren't even my clothes!") But why would you have to break into someone's house to steal their clothes?

In the '80's I had this skirt and blouse that had these 4 inch vertical white and navy stripes and huge red roses scattered around on them. I look back at pictures of me in that outfit now and wonder why I didn't have any friends good enough to tell me what I was doing to myself.

When I was in junior high my shoe strings broke on my only pair of shoes. My Mom made me wear these fluorescent orange shoe strings to school. My life has not been the same since.

One morning I got dressed in the dark and didn't realize until I was at work that I had worn pants with vertical pinstripes with a shirt that had horizontal stripes. It was hideous.
