Some credit George Voorhees as the first Ronald McDonald when he played the role of the clown at a local McDonalds in Los Angeles. But when Voorhees' homosexuality became public knowledge, he was forbidden from playing the role.

Others, such as the corporation itself claim that Willard Scott invented the character when he first played the role in 1963.

Over the years since the introduction of Ronald McDonald in the early 1960s, several men have played the role of Ronald McDonald;
-Willard Scott - 1963–1966
-Bob Brando, after 1966
-King Moody, 1970s
-Geoffrey Giuliano, late 1970s – early 1980s
-Squire Fridell, 1984 to 1991
-Jack Doepke, early 1990's
-Joe Maggard, mid-1990s

I'm not sure who's currently playing the role of McDonald though.
