how do I find the serial number on the modem?

I am trying to fill our the rebate card.

Look at the manual - there should be a reference section for AT commands, and usually one of these will return a serial number. You need to use hyperterminal or some other method to establish a connection with your modem. For a US Robotics Courier 56K, the at command to return the serial is ~~~~ (four tildes).
