Give her something she likes that's not over the top. Why would your fiancee be jealous? Sounds weird to me, as long as you get a gift for her on her birthday, that doesn't make sense unless she thinks you fancy her sister or she's a total brat who thinks that noone else deserves a gift. Either way it's a bit strange.

gift card to her favorite store or restaurant, CD of her favorite band, DVD of her favorite movie genre ; something casual and impersonal, but still thoughtful. See it like buying something for a male friend. The same concept applies. If you still have trouble, ask your fiancee. She might have a better idea of what kind of stores, restaurants, music, movies, etc that she likes.

Something not sexy or romantic... Something for her bedroom, a scarf, a filofax, bag, something like that. Generally a present that will not show something more than friendship. It's better to ask your fiancee for hen opinion or better to go together for shopping for her sister :)

You don't really have to give her a gift. But, if you feel like you must you should really ask your girlfriend, because if you don't consult her you might make her jealous.
