Ah! Sounds like my Mother in law. I bought her a gift certificate to a Elizabeth Arden for a day of pampering a few years ago. She still raves about it!
Patricia has the best answer- I think tickets to the theatre would be fantastic if she's into something like that! It would be great way to spend time together too.

If she has everything, don't buy anything but perhaps do something special about the things she likes. Write a nice birthday card to her, make a surprise with the house full of light candles, cook something for her, invite some of her friends as well and make a great party. Or an intimate party just the 2 with eating and photos and talking about nice moments you share in the past. Or if is a great party with friends, you could make it extra special with some life music, or making the party in a special place, restaurant, outside... If you still feel you need to buy something, maybe you have the confidence to simply ask her, or go shopping together...or buy something different, a cat, a ticket to an event...whatever you don't put on a shelve! I gave my mother a trip to Australia for her birthday. All expenses paid........she was delighted. For someone who has everything, I feel that a paid vacation is the best gift. Who wouldn't need a vacation anyway?

Make something even if you think you're too old for it, it will make her of the old days when you were 4 and brought her a picture of a rainbow...whenever in doubt make something crafty!

think about giving her a gift that is unusual...like a nice piece of art. but i also like the month of housekeeping. a day at the spa.
