The path to image has not been found, make sure you type it correctly either as a relative path or absolute. EG if your image is a folder called pictures the path would be

http://www.yoursite,com/pictures/yourpicture.gif
or /pictures/yourpicture.gif

and refresh your page to clear the cache

It means that for whatever reasons, your browser could not get the complete image file. This can be due to a bad connection, or a bad original file, or any number of other things. The main thing is: it's not on your end, so there isn't much you can do, except hit the refresh button or try the image again later.

I'm assuming you're talking about an image on a web page. It usually means the image file is missing, or the link to the image is incorrectly formatted in the web page.
