Not all birth defects are "treatable" pre-pregnancy.
The most common measure to take, though, is to make sure the mother is getting enough folate, or folic acid, in her diet. This can come from prenatal vitamins or an increase in foods rich in folate.

Start taking folic acid right away, if you smoke, quit. If you drink, quit or reduce intake drastically. Don't do drugs - a definite no-no.
Not all birth defects can be avoided as it could be due to genetics. Research any birth defect in both your families, to see the posibilities of you having a child with defects...

Take folic acid, avoid intake of fish high in mercury, avoid cat litter (toxoplasmosis)
