programmers usually know about a lot of techie stuff
and a lot about different softwares and programming languages
they usually program software applications or web applications
like me, for example, i am a php programmer
i program/develop web sites/applications
i can also make desktop applications using other programming languages
and no, programmers aren't confused on the web
they usually know how it works

I'm a little bit of a programmer, set your mind to it and you can make any program you want to do anything.. and why should anyone be confused about the web...although most programmers don't add anything to do with the web in their programs.
