Title. Director, Producer, and/or Writer. Medium. Distributor, Year.
The title is underlined.

The title of a video would be italicized. It's like the title of a book.

It's underlined. And add if it's a videocassette or DVD before the name of the distributor.
Title (underlined). Dir. Name. Per. List lead actor(s). Videocassette. Distributor, #### (year released).
