The Dead Alive movie is by far the goriest(but kind of funny, check out the dinner table scene)
The most disturbing would have to be Audition or Ichi the Killer(both great Japanese films) Oldboy has a really hard to watch scene involving the main character and his own tongue
As for suspense, I love the original Halloween. The John Carpenter soundtrack makes the movie for me. I'll be curious to see what Rob Zombie can do with it when he releases a re-make this summer.

The Grudge- couldn't sleep right for weeks. The scene were the 'thing' calls on the cell phone and then rings the door bell and noone is there then crawls up into the girls bed form the bottom... WAY TO FREAKY! The Ring- that twisted face of the girl when she comes out of the T.V. in the apartment is just horrifying! The Amittyville (sp?) Horror- some of the scenes just have way too much possibility to be real. Faces of Death- the one I saw showed a puppy being purchased, then taken to someones home and hacked up... I was only 13 when I saw it.. terrible!! Not so much scary, but over the top gory- Sin City. Thank goodness at least most of it wads in black and white. Even so, it was still very very gory.

I warn you though, if anybdoy saws that "Clawed" was a horror movie, ill hunt you down and murder you!! it was the STUPIDEST movie EVER, its sounded cool, but i..i...i was more disappointed than 'The Ring' and 'The Blair Witch project' PUT TOGETHER!!!!

I've never seen I Spit On Your Grave, but from what I've heard so far, maybe I don't want to. :]
I'm an avid horror movie lover, indeed, but the one movie that disturbes me the most is called: Haute Tension (High Tension).
It definitely may not be as gruesome as some of the other movies mentioned, but for some reason it just makes me sick to my stomach and believe me, I can stand a lot of things.

About 15 or 20 years ago, I saw a movie called, "I Spit on your Grave" at the drive-in. It was horrible - rape, dismemberment, boat propeller deaths, guy got his weenie cut off...an author is in a cabin in the woods writing - she is violently raped & sodomized by 4 or 5 creeps. She gets every one of them back in the awfulest ways! I have never been able to locate the movie - anyone seen it?
Also a more recent movie called "Vulgar" (I think?) was also pretty gross - more rape, but this time it was violent same sex rape - these guys tore this guy up! He gets revenge!
