If your breast reduction is deemed medically necessary it "should" be covered by insurance. (Mine paid for my surgery 100%)
You could ask some women on this site that have that insurance who had a breast reduction if it was covered by it or not: http://www.breasthealthonline.com

As for off work, I used some leave time and short term disability time since you need two weeks off of work (or more if your job is very active)

If it's for cosmetic reasons, no. OHIP won't cover it.
You don't need to claim EI (insurance) you're supposed to be on medical leave if you're employed.
