I get this frequently, both on my home & work computers. Often a refresh of the page will correct the problem and the display will show. Does this mean I am missing any extensions, plugins or add-ons? Also, a friend frequently sends me e-mails which contain attachments that never display the photo, only the text.
Thanks!

that usually happens w/photo files. sometimes people delete the photos but they don't update their pages, so when the page loads it tries to find the file but it's not there. other times as you mentioned a simple refresh will do the trick in this case is the broadband connection that times out. in the case of the emails there must be a filter from your emailserver that is preventing you to view the photos, this can be corrected using options from your email provider righ in your window, look for it. in the case of hotmail there's a button that asks if you want to display photos or not. the default is not to display them. one of those should work.

It means an image should be displayed but is not available.
It can mean that the website you are visiting has moved the image, but sometimes it can be something on your system blocking the image - Sometimes deliberately because it is set to block certain ads, and sometimes because you have security software that is being over-zealous.
