I need information regarding NCAA licencing fees.?

I would like answers to the following:

1. How does one obtain the right to legally produce apparel with NCAA logos?

2. What are the fees? (Is there a per-item fee and a yearly fee?

3. Are there any special requirements to be met in order to obtain rights to produce licensed merchandise?
