usually when your phone asks for a code, if u get it wrong a few times it asks for a "pac" code. To get this code u need to contact your network provider. if again the "pac" code is entered incorrect a few times, u need a "puk" code, if u enter the wrong number 4 this, it completely blocks your sim! contact your network!!

Frogg's right.
You've set a pin number on your sim and input it incorrectly 10 times.
Contact your service provider and they will give you the PUK code to unlock the sim.
Don't ask for a PAC code. That's used for porting your number to another network.
