If 'dilemma' is the word you have in mind, it means a situation where one has two or more difficult, unfavorable choices to decide from, all of them being equally well-balanced.
A related expression that we come across is 'on the horns of a dilemma' which probably explains the word more graphically, where there are two difficult choices.

I think the best thing to do is to talk to your step-sister.. And if you tell her you've thought a lot about it.. I'm sure she'll give you her blessing for the name Natalie.. It's really something you need to discuss with her though, not any one else.. Because you won't know how she truly feels until you talk with her.. & I guess it always depends on the situation of the adoption.. Maybe you could use Natalie as a middle name instead of first.. You would still have the name you've got your heart set on.. & your step sister.. wouldn't have to call her Natalie, but know that it's still her middle name.. The decision is truly yours.. but I Hope it all works out!! If you are trying to approach her, start talking to her about babies.. & tell her how its hard to decide on a name.. & then tell her you have some names picked out & and you're really considering Natalie.. and see what she says, and ask her how she feels about it..

Thats definately a touchy situation. I think that maybe you should choose a different name because it may bring back memories for your sister. Even if you talk to your sister about it and if she says she doesn't have a problem with it I think you need to still take her feelings into account and not use the name Natalie at all.
