Strumming or fretboard? I play the uke (no kidding) and for certain chords, I have to position my wrist in a weird way and until I get used to playing those chords, it hurts.
I'm also a computer graphic artist (9+ hrs/ day on a mac) and my right wrist sometimes is tender and that bothers me to strum (uke strumming comes from the wrist).

Wrist and fingers, even with callousy fingertips. Thin strings hurt more. When all that happens, I switch over to playing slide.

But in the serious side - my sister DOES PLAY THE GUITAR!!! yes her wrist hurts...as well as her thumb.....but id go for what the others have answered! wakokok!!!!

Yeah, the joints in my fingers to. I play allot of Jimmy Page's stuff and I don't know how he did it every night for hours on end.
