An affidavit is a formal written statement we use in law enforcement. There are different forms of "affidavit's" to include those made from a "suspect," a "witness," and even from an officer.

In my particular job, if I need a search warrant issued by a judge, I have to provide the judge with an "Affidavit in Support of a Warrant."

Hope this helps...best wishes!
