Common practice is to give a WRITTEN letter of resignation to your immediate supervisor with at least two weeks notice, one month is better, especially if your in management or medicine. Send a copy via certified mail return receipt requested to the human resources department. This is so there is absolutely no question the they received it. You don't want to burn your bridges behind you and you want to leave a good impression as someone who is courteous and considerate in case you ever want to return to your that company in the future.

Depends on the company. Some want you to work out a notice but some want you to leave immediately. If you already have another job lined up, you can leave when you want to, but it is best to offer to work out a notice. You never know when you may have to ask them for a reference.

I don't get it? You give your immediate supervisor written notice and a copy to the HR office. Don't forget to send HR a copy, my boss left the one I gave her on her desk and went on vaction, when my last day came HR said I didn't give two week notice.
