I have been caught by this annoying and aggressive move so many times when playing online, and I have never found a satisfactory answer: consequently, I end up making a mess of the game. If 1. e4 e5 2. Qh5, how should Black answer? or if playing as White?

That's actually not a very good move. It exposes the Queen too early in the game. The best tactic is to use attacks against the Queen to gain tempo.

1.e4 e5
2.Qh5 Nc6
3.Bc4 g6
4.Qf3 Nf6

And now the checkmate threat at f7 is over, and white is behind in development.

Well I agree with everyone that 1.e4 e5 2.Qh5 is a suspect opening choice and I also agree with the others that 2.Nc6 is the best response but GM Hikaru Nakamura has opened games with 1.e4 e5
2.Qh5
so this opening should not be underestimated.

1.e4 e5 2.Qh5 is a complete violation of chess principles that
must be immediately punished! I tell any young player who will listen to me (and they have hard heads and don't like to hear the truth) not to play this opening why? because the opponent will punish them with rapid and efficient development bring out all his army cost them time retreating their Queen and if their opponent is a very strong player he or she may prepare a very painful trap they will not be able to escape!

One plan I would suggest is to play 2.Nc6 and if he plays say 3.Bc4 then g6 and then if 4.Qf3 you now have the choice between Nf6 or even better f5! If White takes the f5 pawn, Black gains control of d5 and can quickly build ideal pawn center d5 and e5. If White is greedy he'll be overwhelmed.

1.e4 e5 2.Qh5 Nc6 3.Lc4 d5 is an optimistic plan..after 4.Bxd5 you're a pawn down.

Better is 3...g6 4.Qf3 Nf6 5.Ne2 Bg7 6.Nbc3 d6 7.d3 Bg4 and white made 3 queen moves...so it's not a very bad move, but you do lose time. If you don't want to play g6 there's always 3...Qe7.

There are players who employ these ridiculous openings and I punish them everytime. With the start of a chess game, you are trying to establish targets. With 2. Qh5, White has already offered a prime target. He will lose tempi by trying to find a safe square for it. I will not offer the moves, think for yourself.

i agree with the others that this is actually a weak move. Kc3 or d3 will do fine. then attack his queen while advancing your own development.
