I know such a person.
Using his first name calls attention to himself but separates him at the same time.
I think he thought he presented himself as better than while suspecting that he was actually worse than those around him.
It annoys people because we can feel the rejection and see the arrogance.

I think it's lame, reminds me of that Seinfield episode with "The Jimmy" who refers to himself in the third person. Pretty soon George was doing it too. Funny to watch, but can't imagine being around someone who actually did that all the time.
