Normally the closure of the neural tube occurs around the 30th day after fertilization. However, if something interferes and the tube fails to close properly, a neural tube defect will occur. Among the most common tube defects are anencephaly, encephalocele, and spina bifida. The incidence of neural tube defects is 2.6 in 1,000 worldwide.
Pregnant women taking medication for epilepsy have a higher chance of having a child with a neural tube defect. Research has shown that women with folic acid deficiences also have a higher chance of having a child with a neural tube defect, but this is only one factor. Taking folic acid does not completely negate the risk of neural tube problems, but it does significantly reduce the risk.

It's important that you don't blame yourself or believe that there is something you could have done differently to prevent this from happening. Unfortunately there is no cure for anencephaly.
The condition is called an open neural tube defect, or ONTD. According to the Yale Medical Group:
"ONTDs happen to couples without a prior family history of these defects in over 95 percent of the cases. ONTDs result from a combination of genes inherited from both parents, coupled with environmental factors. For this reason, ONTDs are considered multifactorial traits, meaning "many factors," both genetic and environmental, contribute to their occurrence."
And regarding future pregnancies they state the following:
"Once a child has been born with an ONTD in the family, the chance for an ONTD to happen again is increased to 4 percent to 10 percent. Genetic counseling may be recommended by the physician to discuss the risk of recurrence in a future pregnancy as well as vitamin therapy (a prescription for folic acid) that can decrease the recurrence for ONTDs. Extra folic acid, a B vitamin, if taken one to two months prior to conception and throughout the first trimester of pregnancy, has been found to decrease the reoccurrence of ONTDs, for couples who have had a previous child with an ONTD."
