There is nothing wrong or "dirty" about it...although it is a bit messy. Now, as far as being safe...I am assuming you are practicing safe sex. Always wear a condom...although you will be dealing with menstrual blood so it is dangerous as far as you have a higher risk of transmitting sexually transmitted diseases.
I would say, if you are going to do it, I would hope you are with someone you know is clean of all diseases and that you are as well. If you are taking all the precautions and are comfortable with it, then go ahead. There is nothing wrong with it.

The biggest impact would be on whoever does the laundry. Those bloodstains can be a *****. Other than that, it's perfectly safe. In fact, orgasming could ease her cramps, so in her case at least, it has benefits.

theres nothing wrong with it...it is messy, though. it actually relieves cramps for women (just be fairly gentle), but it's supposed to be very good for women...i've tried it and i liked it :-) *i'm a woman*
