About your mind I don't know, but you can be sweet anytime you want to. The mind is always reeling and thinking about stuff. Especially when you have a vivid imagination. Just cause you had a sick or dirty thought doesn't mean that you're mentaly unstable. Look at writers and horror movies, I bet some have spouses and they're sweet to them.

You didn't give enough details for anyone to give any kind of answer.
That said: if you're asking a question to a bunch of strangers like: am i sick, chances are there is a mental defense mechanism that is telling you that you ARE sick. Seek medical attention ASAP. You've been giving honesty, after this it's your own stupidity that will keep you from the help you need.
