the problem, intervention, and evaluation of outcome. example ms. gregg has a speech deficit (if this is for nursing school i highly reccomend the you look up the accompanying nursing diagnosis). a couple interventions would be for her to work with a speech therapist to improve speech ability and to provide a communication board with letters and symbols for her to use temporarily. after this has been in progress after a period of time, usually when medicare is going to stop paying, ms greggs progress will be evaluated to see if what is being done is effective or if other interventions need to be in place.

I would expect from a care plan: When she suffered from the stroke, the level of her paralysis and her speech. SInce she is well able to understand communication, and her level of activity before her stroke, I would expect that in the care plan she would have included a multi-disciplinary team for her rehabilitation including a physical therapist, speech therapist, occupational therapist and a home health nurse and aide. All her immediate needs would be met as to her care, and also her needs for "getting back on her feet" and with her schedule being so full with the "team" she would be getting plenty of social activity in a matter of speaking, and also to include as much as possible her friends and family in her recouperation. She is still young and considering her history, she seems to be a very good candidate for a optimistically positive outcome.
