Heroin...but I can't help you with the side effects, but have heard that a person has to be 'weaned' off it and can't just quit 'cold turkey' or it could kill them.
I found this information from the third link below...
Effects

The inital idea was to prevent depression, until the side effects were found. Heroin is highly addictive. Withdrawal symptoms are extremely painful both physically and mentally. Former user may experience anxiety, sleeplessneess, desperation, addiction, constipation and loss of appetite. Effects are worse if mixed with other drugs. Skin infections can result because of injections. It could increase the spread of HIV through sharing needles with others. Heroin overdose can induce sleep, or if taken in large amounts, the addict may go into a coma or ultimately die.

love can be the worst...addiction to a person can be almost as bad as the traditional drugs
the side effects of lovesickness can be: depression, loss of appetite, mood swings, dizziness (once in a while), fatigue..oh boy do i get tired alot. some people commit suicide from it.
i say this because the emotional parts of it is as bad as the physical effects in drugs like crack and alcohol

on a side note: as far as traditional any smoking is nasty to me...i wanna leave the room. thats one reason i dont like to be around my father. everytime he took a puff of that cigarette when i was with him, i held my breath. it is also worse for me because the guy i have a thing for smokes..and alot of times i can picture his lungs getting smaller and smaller. worrys the hell out of me...i hope someday when i see him again i can convince him to stop but without ruining his privacy (he doesnt want people to know he smokes, they might think of him different)
oh we used to go this club and there were people smoking cigars. eww! those were the worst. they stink too...

Alcohol is categorized as a drug. It is a big killer as well. If abused, alcohol can lead to liver, kidney, heart, and nervous system failures. Pserosis of the liver is a huge side effect of abuse. Drunk driving kills as well. Alcohol is a highly addictive drug. I know, for I am a recovering alcoholic who nearly lost his life, due to it's effects. And since it is available at nearly every corner, and legal to purchase, if of age, I would say it's one of the most dangerous of drugs.

I think crack is the worst drug ever. But then again all drugs are no good and have some dangerous side effects.

For me i would say skunk.makes u spin n go really hot.these are made 10 times worse if you're drinking.
ALL drugs are bad n will send you "off the rails".I experimented with drugs when i was 16 and i just got arrested. They mess with your mind and destroy your personality.Luckily i saw sense and stopped but many people don't and end up addicted and in the gutter.

Nicotine. Just go to any Narcotics Anonymous meeting and ask any of the recovering addicts which is the hardest drug to quit and most will give you the same answer, that is, if they are/were smokers.

Methamphetamine hydrochloride, or "ice" sounds like a pretty messed-up addiction to me. Any form of drug abuse is dumb. Even legal drugs like nicotine and alcohol can be dangerous killers if you let them control you instead of vice-versa.

Methamphetamine. Without a doubt. Side affects? Rotten teeth,
violent behavior, total disregard for one's well-being, total disregard for the feelings and/or well-being of others, destuction
of families.
I have seen meth in action, it is SATAN.

Nicotine-has been proven the most addictive and widely abused drug. Most people know someone who is sick or has died from using it.
