it has happened but it is not very often, because things are planned in advance. they are scripted, more often than not what happens is one of the veterans will be wrestling one of the newbies and the newbie will screw up or "no sell". when this happens the vet has a tendency to "stiff" the newbie. what this means is that the vet will make his body more rigid which in turn makes the move hurt more, no injury is created but the newbie learns not to screw up again.
one fight that was real was batista injuring mark henry, batista has openly admitted to purposefully injuring mark henry. he did this because mark henry was well to dumb to know when enough was enough and injured a few other wrestlers including batista. all of the fights you saw between them when batista came back were real.

Of course, its called a shoot. Probably the most famous recently was between Matt Hardy and Adam Copeland (Edge) over an affair with Amy Dumas (Lita). The shoot was latter worked into an angel, but Matt Hardy was pulled out of Money in the Bank '05 because he might have started a real fight with Adam. Another shoot that I know of involved Perry Saturn shooting on a jobber at a dark match (i think it was, either that or a house show) after the jobber botched a move. You might remember Saturn as that dude with the mop a few years back. Word is, he was given that gimmick as punishment for the shoot.

Of course real fights have broken out (Called a shoot). More so in the olden days of pro wrestling. In the olden days Promoters almost always made their champions guys that could legitimately shoot in case a rival promoter tried to double cross them, as it was common for inter promotional matches to occur on cards. In the modern era some examples of shoots include Bob Holly beating up Rene Dupre at ringside because Dupre got a ticket with Holly's rent a car. In addition several wrestlers have said that matches come close to shoots more often than people know as tempers have a way of flaring.

Yeah, at the end of ECW: One Night Stand, JBL brawled with the Blue Meanie, bloodying him up and injuring him on purpose.
Billy Kidman injured Paul London with a Shooting Star Press on a Smackdown episode.
There are many accidents that happen, but legit fights are rare in a show.

The Blue Meanie and JBL actually started throwing shots at each other in a Battle Royal. Apparently they really hate each other. JBL actually cut the guy.
Umaga really kicked Steve-O hard after he started laughing at the Samoan drop he received.
Batista and Booker broke out in a fight at a photo shoot.

well i dun think so since it is fake, no matter how hard they try, accidents will happen as this is a dangerous sport, and they already know all the dangers they are going to face, they will be mad of course for being injured but that cannot be helped u know

Yes in 1988 wrestler "Bruiser Brody" was stabbed to death in the showers by "The Invader". The reason for the stabbing is not clear but the murderer was acquitted after claiming self-defense (some people claim it was not self-defense but a mix of money and personal problems).

I actually think that the brawl that broke out with Bret Hart and Mr. McMahon was real. Cuz' Bret Heart swears to this day that Mr. McMahon changed the plans without telling him. And he still hasn't come back yet.

I think one time JBL bloodied someone really bad at ECW's One night stand.
And I think New Jack kept beating someone up after they were injured badly and he didn't care.
