My left ear, always; I'm left-handed. Just after I got my cartilage pierced in that ear though, I held it up to my right ear for a while.

Begin with the right ear. If I am on the phone for several minutes, I start switching back and forth from right to left because my ear starts burning.

First my right ear, then my arm gets tired so I switch to my left ear. I don't find that all that comfortable so soon after I'm switching back to my right ear. This goes on during the whole conversation.
