The answer is D. They could be male (as in E), but they would still need to have 4 X chromosomes, three of which become Barr bodies. D is correct in all circumstances.

This doesn't answer your question, but I just wanted to say that I don't believe people with Down syndrome "suffer". My son has Down syndrome and he certainly doesn't seem to be suffering.
