If you don't put antifreeze in the block will freeze and make a whole or blow out a freeze plug. A connecting rod going through the side of the block because you did not put oil in.

In diesel engines if you don't use a water conditioner a bubble can be trapped and ping a whole through the water jacket into the combustion chamber.

The only time I've seen a hole in an engine block is from misuse.

I don't understand what you want to know. Did someone tell you there is a hole in the block? Better information will get you better answers.

Throwing a rod. Overheating and driving through a cold puddle.

Basically either through catastrophic mechanical failure, or through extremely rapid temperature changes. (Though the latter is more likely to cause the block to CRACK)

On purpose, use a drill. By accident....connecting rod being driven by the crank usually makes a nice hole.
