I am going to be babysitting my neighbors daughter for awhile and I need to know what kind of legal forms should I have her sign saying she will not sue me if we got in an accident. I will have to drive her daughter at least 4 out of 5 days of the week. Does anyone know where I can find these documents online? Also need a document allowing me to seek medical care for her in case of emergency. Thanks!

Her parents need to provide you with a Limited Power of Attorney -- one that authorizes you to take her to/from her activities, to take her to the doctor or for medical care too.

They also need to provide you with PROOF of INSURANCE on their daughter as well.

For these documents -- you need to be able to have a local attorney to help you prepare them, or the legal services (if they have it) at where they work.

IF this is a child of a Military Servicemember, they already will have prepared these documents as part of their deployment preparations.

I agree with sglmom. You need a limited Power of Attorney. In my state, you can download them from your state court judicial website. You can also buy them in a stationary store. You shouldn't need an atty for this. I don't know where you are, but try (name of your state here)findlaw.com or www.__________statejudiciary.com. See if there's a link for forms. A copy of the child's insurance card is also a very good suggestion. I 'd give sglmom the points, btw. (when I preview this I lose the ".com" after the sites)

Why don`t you ask her to give you the child`s Health Card in case you need to get the child in for medical attention but l don`t think your gonna need any forms to sign and your only babysitting so don`t worry about any forms. l never even heard of lt before.
