The short answer - it doesn't. Butch is often used as a derogatory term for lesbians and masculine-looking females. So that's just what it is - a derogatory term used by people who don't know any better.
A woman who can work on cars and construction is whatever she chooses to be.

Just because those acts are associated with men, and so if a woman does them, she must be butch, we assume.

Of course, it's not necessarily true- I've seen absolutely feminine women to work on cars and other 'masculine' jobs, without losing an ounce of her womanliness.

So, all in all, it's a stereotype.
