Does that mean you can't read or that you can't spell?

It literally means that you are unable to read or write.

That means that you cannot read or write in your native language. Also, you can be illiterate in other languages, but it is not the same thing. There is a difference between being able to read stop signs and grocery lists, and to be able to write themes, projects, and business letters. Most computers have a "Spell Check" that helps with that. However, be SURE you have a word you want to use before you click on Spell Check.

Can't read or write.
Someone may still know how to spell through speaking (possibly)

Generally it tends to mean both, but it's usually applied to reading skills.

Writing is usually ungramatical (is that a real word or did I make it?) or poor spelling.
