Why don't you try asking these questions of your cell phone provider? The rate changes with the company.

To dial US to UK it is 011-44 and drop leading zero of UK phone number. UK to US is 00-1+ number
Phone charges will depend on your provider as said elsewhere

I'm not exactly sure, but metro pcs has it where you can text anyone around the world for 2 dollars a month and it's unlimited text messaging, so you dont get billed per text message you receive or sent out.

All depends on mobile provider.
Check with them before you start - some like o2 - can give you bolt ons where you pay a fee and get texts cheaper.
unsure about codes - don't know of same as landline - check again with supplier they'll tell you.

get the person in the US to register (free) at www.peekamo.com. they can give you the login and password, they the person in the UK can text them from any PC for free (both for the sender and the person in the US that is getting the txt). Free for everyone.
I use it cause my bro's in the UK and it costs him a lot to txt me, so he uses Peekamo.com to text me for free!

it all depnds on the provider, there is no certain fixed rate, the best person to ask is your phone company.
and yes youll need to add the country code and city code to the number before you text them

It varies, on Orange pay as you go, I pay 20p for a UK to US text. You need to use +1 for the US e.g. +14085551234
