You cannot change their genetics. When baby sims grow up to toddlers (and afterward) their genetics genotypes begin to show, so they look like they are part of the family.
The clothing, hair, etc. is random at first (as it was in The Sims 1), but you can have them use a mirror to change their appearance (i.e. hair and makeup). You have to make an adult go downtown to buy some clothes for your toddler if you want a different look. You have to have a dresser so your child sim can change clothes.
I'm not sure if children can use the reward that lets you change your face structure (I can't think of the name of the object, but I think it's a plastic surgery station). It may be usable by adults and elders only.

Well, at first it's random.
Later on you can change its outfits by going to a dresser, and you can change the hair/makeup in the mirror.
Genetics is unchangable though =/

It's random. But once they become a toddler(if you have Glamour Life or Open for Business. I'm not sure which) you can use the mirror to change appearance of said toddler.

I think that if you go to the store, you can buy it new clothes but I think that is about it. You can also change the hair of a child by going to a mirror.
