It is completely your choice. If your fiance wants to claim paternity there is a part of the form that allows him to do just that.

That will be your decision at the hospital. You fill out paperwork for birth certificate. You can actually put ANY name you choose.
however where it asks fathers name._____IT is left blank if he is not there to sign papers that establish paternity.
keep in mind you have choices--- you can either get married before baby arives--- giving you all same last name. Or you may end up not doing so, then school age or before you will continually be referred to as mrs--(ex's last name & childs as well), you may choose to hyphenate- giving child both mom/dad's last name- which can later be dropped to just one,-. This can save court cases for name change later also.
Think it through- dont just automatically give your child the fathers last name- Since he has not married you yet, You may be alone raising this child.....

You can pick! Whatever you put on the birth certificate is what it gets. Some hospitals will only refer to the child as the mothers last name but it can have your fiance's last name if he is the father!

My oldest daughter was born before her mother and I got married. The name that goes on the birth certificate is up too the parents as long as both are consenting to it. If either of you is not then by default your child will have your last name.

You choose the last name for your baby. If you are certain you are going to marry him, give the child his last name. That way, after the wedding you won't have to pay for the child's name change. If you aren't 100% certain you will marry him, you may want to give your baby your last name.

Congratulations to both!! It is totally up to you. When the baby is born you put it's name on the birth certificate so you can decide now which name it will have.

You fill out the last name on the birth certificate. The child will have your last name if you so choose. I recommend this until you are married - for your childs sake.

If you want the baby to have his last name, he will have to sign a paper at the hospital taking legal responsibility and stating that he is the father.

the only thing you guys need to fill out at the hospital (thats how its done in IA) is a parent affidavit. which basically says you aren't married but would like the fathers last name
