Well, River Phoenix died of a Heroin OD, not cocaine or crack and Cobain died from getting too many shotgun pellets in his head, not drugs at all.
An inquest into the mysterious circumstances surrounding the death of fallen Italian cycling hero Marco Pantani has found that he died of a cocaine overdose, the Ansa news agency reported Friday.
"The death of Marco Pantani was caused by acute cocaine intoxication," according to the draft report by doctor Giuseppe Fortuni for prosecutor Paolo Gengarelli.
The report found that there was not enough evidence to support the theory of suicide and that Pantani died as a result of swelling to the brain and lung tissue as a result of cocaine poisoning.
"Acute cocaine intoxication provoked a pulmonary and cerebral oedema (fluid) which were the cause of death," the report concluded. "There are no concrete elements to support the hypothesis of suicide." The complete report is to be published in two weeks, according to Reuters.

Janis Joplin in the 70's, Jimi Hendrix in the 70's, John Belushi in the 80's. These are three of the most famous one's that come to mind. There are many more. Sean Penn's brother, an actor I believe, died of complications from using a multitude of drugs, include crack. He died in the mid 2000's. You would have to look up Sean's biography which I am sure would list the names of his brothers and the one who died from complications of cocaine use.

john belushi- cocaine/ heroin
p.s. not sure about that Cobain thing- proximal cause was self inflicted shotgun blast to the head; also legend has it that heroin was much more his thing than coke, and i never heard of him doing crack cocaine

Cocaine was a part of the reason for Mitch Hedberg's death. The guy was one of the funniest comedians I've ever seen , a real shame.
