Unplug. You need take one big day and organize your life. This is the only way in which you will begin to change. Notice that I said "begin" to change. This is not the cure all. As with all habits, it takes constant practice and willingness to get out of it. But this is a great beginning.
Realize that your life, first of all, can be divided into several categories. And from those categories you make decisions, build projects, and design to-do lists.
Here's a rough sketch of what an organized life could be:
PERSONAL LIFE
Family
Body
Friends
Relationships
Activities for fun
Life goals (marriage, house, kids)
Networks (clubs you belong to, non-professional oganizations, like a soccer club)
PROFESSIONAL LIFE
Job
Coworkers
Business
Education
Networks (professional organizations)
Professional Goals (promotions, better pay, etc...)
From each, create your life. A lot of people do this and it is a great tool to create priorities and set courses of action, and not be swayed by other people's opinions and such.
Relax. Plan your life. Live it. Enjoy it. Forget the world. Live your dreams. There's no time to waste.

This can happen for different reasons. You may have trouble maintaining your boundaries or maybe you're just having symptoms of an attention span disorder, but there are many possiblities and causes.
You need professional help to diagnose the condition and get the right answers to help make your life better.
1) Talk with a school counselor if you're in school.
2) See your doctor and explain the situation.
3) Or if you don't have a doctor, call United Way. They will be able to give you a list of doctors, clinics or hospitals in your area that will be able to see you. You can even ask for doctors that will see you sliding scale fee (or based on your income).
4) If your employed, chances are your job offers a toll-free Employee Assistance Program (EAP or EAR). This number will be free and totally confidential. You will get a set number of free sessions and referrals to help.
Life can get easier. Help is available. Start with these suggestions.
Best to you.

I assume that you mean that you are unable to get things done, because you are distracted by others. One remedy you can try is to start the day with a (short) list of things you MUST do today, and stick to it. To make it more powerful, add deadlines to it, like "finish this report by 10 a.m." End of the day, see how well you did, and work on getting a 100% score. Do not make a long list, you will feel bad seeing the number of undone things. You need to prove to yourself that you can do it, you can even start with just one item.

Cut down on the amount of things that are there to distract you in your home space like television or Internet. Try learning progressive relaxation or meditation as well in order to better centre yourself.

There is no need to worry. You can learn more from others by interacting them emotionally. You should try to write poetry etc.
