YES!! He could possibly have testicular torsion, which means the veins leading to his testicles are twisted and he's not getting enough blood to his testicle. If he doesn't go immediately, he could lose it. It happens fast, too!! The time it must've taken to type your question + the trip time was enough to probably have lost it. Good luck.
Here are the symptoms of torsion:
* Sudden onset of severe pain in one testicle, with or without a previous predisposing event
* Swelling within one side of the scrotum (scrotal swelling)
* Nausea or vomiting
* Light headedness

You should at the very least call his doctor. I would take him to the emergency room to rule out a testicular torsion (medical/surgical emergency!). It could be a varicocele, but there is no way to know without having a doctor take a look at it. At the very least they'll give him pain medication and peace of mind. Don't hesitate, take him to the Emergency Room if his doctor is not available.

Have him stop walking and put a bag of ice on it, to see if the swelling goes down. You have to rest and stop doing things to make it better. If the swelling does not go down, then go to the doctor or ER.

If you have to ask if you should take him to the E.R. then take him, don't wait for people to tell you to take him, don't play around with anyones health. In the worst case the doctor will tell you it is nothing. Nothing happened. If it is something they can fix it and tell you what to do. In any case it is best to go don't ask just do.

He may have hurt his testicles & it has got inflamed. If that is the case he should take anti inflamatory like Ibuprofen & give some heat (Mild) to scrottal sac. If it is not the case than it must be filarial scrotum. See any surgeon.

If he has not already seen a doctor, do it now. The best choice would be an outpatient clinic in a hospital or the emergency room.

Could be a hernia or a cyst. Better take him in. Have you uh...attempted to relieve the pressure yet. Sometimes an orgasm can knock things back into place.

Take him to the ER. He may have a constricted testicle that, if not treated, may have to be removed, and if bad enough they'll take both.

I would get it checked out. You don't want to mess with something like that.
