Decide what you want to know

start with demographic information
suburb, age, income range etc

use multi choice questions where you can to make it easy to quantify answers.

Make sure the easier 'stem' questions come first, then move on to more complex or 'branch' questions.

Allow the answerer space to comment as you may get useful information or bites that can be added in the report once the information is collated.

Limit of 10-20 questions so the answerer does not feel like they are sitting an exam.
