Bats fly out of caves in dense bunches at great speed. Bats flying out of such an unpleasant place as Hell would presumably be flying at even greater speed in even more chaotic fashion. Hence "like a bat out of Hell" infers speed, recklessness, and chaos.

to move or get out of a situation in a quick hurry. Like if a bat were trying to get out of hell how fast it would be moving.
