what are they asking for when they ask for "Reference Type" next to the reference's name? Are they inquiring what the relationship is that I have with this person?

Yes. It is best to give strong references i.e. old bosses, or supervisors; although personal references work as well

They want to know their relationship to you. Whether it be mother or friend. Will this person be honest in what they say about you or make up something.

yes. and be sure to fill in the address and phone number the person can be reached at. they will want to talk to them.
