An "outie" is caused by umbilical hernia. Umbilical hernia is a congenital malformation that babies are prone to during fetal development during the process by which the abdominal organs form outside the abdominal cavity, then later return into the abdomen through an opening which will become the umbilicus (the navel). A child with an umbilical hernia will not necessarily develop an outie. It appears to occur more frequently in boys.

It has something to do with the way that they clamp the cord...I have 8 kids and 2 of them have outties....The ones with the innies had their cord clamped close and the ones withe the outties had about 2in clearence and its the way it heals in the first ten days before they drop off.....Am not sure but it has been a question that has baffled me for 16 yrs and i came to the conclusion that, that is the best answer there is.....

It does NOT matter on how the umbilical cord is cut, it is just genetic. However it comes out, is how it comes out.
