It refers to people doing extreme things to their bodies, such as piercings, tongue splitting (yikes), etc. I once saw a guy on TV who wanted to become a cat, so he got whiskers "implants" as well as tattoos all over his body to simulate a cat´s fur.

Some people actually do some stuff like that for spiritual/religious reasons, but i´d say most people in western society do it for easthetic reasons.

Do a search on wikipedia and you´ll find the craziest things explained...ugh...
