My opinion. Yes, he probably has an ear infection. Fluid collects behind the eardrum and the tube (eustachian tube) becomes clogged and the tube will not drain the fluids away. If this fluid becomes infected--an earache will develop. He need to see a doctor and he will order antibiotics.
A family doctor can do this. You do not need a specialist. If it is something serious--the family doctor will send you to one.

Probably an ear infection. Put a couple drops of peroxide in his ear at night until it goes away.

You must first ascertain what kind of pain he feels. There are several reason why it would hurt. When in doubt, take him to an Otologist (ear doctor).
